A strike on Moscow would kill 1.4 million people and seriously injure another 3.7 million.
A nuclear attack on Beijing would kill 1.5 million people and injure 3.3 million.
A nuclear attack on Pyongyang will kill 1.3 million people and leave 1.1 million injured.
The main harmful factors:

Blast wave (108.95 km²). destruction and fires;
Thermal radiation (339.57 km²). people will get third degree burns.
Glazing damage (1385 km²). destruction of windows.
